Our Community Advisory Board (CAB) and Partners serve as a bridge between the HEARD research project and the communities they impact, ensuring that community voices are heard and considered in decision-making processes. The board provides valuable feedback, guidance, and recommendations to help align initiatives with the needs and concerns of the community. Its purpose is to foster collaboration, build trust, and enhance the effectiveness and relevance of programs or policies.

President and CEO | Self-Help for the Elderly

Ms. Chung is the President and CEO of Self-Help for the Elderly, an organization that operates many programs for hospice and Alzheimer’s patients. Ms. Chung’s organization provides care to seniors with dementia. Ms. Chung is also on the API Council and the Co-Chair of the San Francisco Palliative Care Work Group.

Family Care Specialist | Alzheimer’s Association

Ms. Green is a Family Care Specialist with the Alzheimer’s Association. Her work involves giving support to patients and families who have been affected by Alzheimer’s Disease. Sandra has worked in the long-term care industry for over 30 years.

Chinese Community Ambassador | Chinese American Coalition for Compassionate Care

Ms. Huang is a part of the Chinese American Coalition for Compassionate Care and the San Francisco Palliative Care Workgroup as a Chinese Community Ambassador. Her career has centered around hospice and palliative care, specifically as it relates to serving the Chinese American community.

Ms. Lewis is a retired Geriatric Nurse Practitioner. She worked in nursing homes caring for patients with MCI and dementia, in hospice, and with end-of-life care until 2012. She is an active volunteer in her church and community and belongs to an Alzheimer's support group.

Morela Madriz-Ray

Director of Community Education & Awareness | Homebridge

Dr. Madriz-Ray is a physician trained in Venezuela with more than 30 years of experience in health care and education. She has worked at Homebridge based in San Francisco since 2010 developing curricula in English and Spanish and training caregivers throughout California. She also collaborates with the San Francisco Palliative Care Workgroup. Morela is driven by a deep passion for medicine, health care, and community service.

Ms. Mattero is the Healthy Aging & Disability Program Manager at Mission Neighborhood Health Center, Inc. where she oversees the day-to-day engagement within community.

Professor of Medicine | Department of Medicine/Divisions of Hospital Medicine and Geriatrics, University of California, San Francisco

Dr. Moody is a professor of medicine in the Divisions of Hospital Medicine and Geriatrics at UCSF. Dr. Moody holds various academic positions, including Medical Director of the Palliative Care Service at the Zuckerberg San Francisco General Hospital and Trauma Center. She has been a leader in health disparities and palliative care related to older adults.

Dr. Perissinotto is the CEO of Sabio Health, a home-based palliative care program. She is also a Professor in the Division of Geriatrics, Department of Medicine at UCSF. Previously, she worked as a hospice physician and spent over 10 years with the UCSF Care at Home program. Dr. Perissinotto is passionate about working with diverse communities, particularly Spanish-speaking communities.
